By submitting this form, I agree to the following terms & conditions:

I understand I must compete from the registered boat with the installed core items purchased from Radioworld Inc. for the duration of an eligible tournament to qualify for a payout.

I will meet all qualification guidelines outlined with each sanctioned tournament trail and program rules and conditions of the Radioworld Edge Performance Payouts program for each contingency paying event.

I will place the official Radioworld Edge Performance Payouts decal on the driver’s side windshield of the registered boat.

I will maintain high standards of sportsmanship, courtesy, safety, and conservation.

I will abide by all local and federal laws and regulations, including but not limited to possessing a valid sport fishing license and boat operators’ proficiency card.

I will complete, sign, and return this form prior to competing.

I understand that Radioworld Inc.’s decision is final in all matters determining the eligibility and claims of the Radioworld Edge Performance Payouts program.

I understand to allow 4-6 weeks for payment of qualified claims.

For each Radioworld Edge Performance Payouts paying event I compete in, I will submit a photograph time-stamped during competition hours in my eligible boat, with my purchased electronics clearly displayed.

I agree to allow the use of my name, photography, and/or likeness by Radioworld Inc. for advertising and publicity purposes without further compensation by participating in the Radioworld Edge Performance Payouts program.

I agree that, if I should win an event, I will submit verifiable results, or a copy of winnings cheque received.
